EnzymeimmunoassayforthequalitativeandquantitativedeterminationofIgGantibodiesagainstmyeloperoxidase(MPO)inhumanserum.Anti-neutrophilcytoplasmicantibodies(ANCA),originallyidentifiedbyimmunofluorescenceassays(IFA),aredirectedagainstcytoplasmiccomponentsofneutrophilgranulocytesandmonocytes.TheyhaveproventobeausefulSEROlogicMarkerforanumberofsystemic,autoimmunemediatedvasculitides.Aperinuclear(p-ANCA)stainingpatternoftheneutrophilsubstrateisindicativeforautoantibodiesagainstmyeloperoxidase(MPO),a145kDalarge,cationic,covalentlylinkedalpha2-beta2complex.MPOautoantibodiesaremostcloselyassociatedwithoneformoranotherofmicroscopicpolyangitis.Theyalsooccurinpatientswithpolyangitisoverlapsyndromewithoutrenalinvolvement,idiopathicorvasculitisassociatedcrescenticglomerulonephritis,classicpolyarteritisnodosaandChurg-Strausssyndrome.ThetiterofMPOantibodieswasfoundtoreflectflaresandremissionsofthesediseasesinindividualpatients.
