TheABIlityofleukocytestoextravasatefromthevasculaturetothetissuespaceisafundamentalfeatureofthenormalimmunesystem.Severalcelladhesionmoleculesplayakeyroleinthiscomplexprocessofinitialandstableadhesionanddiapedesisofleukocytesacrosstheendothelialbarrier.Vascularadhesionprotein-1(VAP-1)isoneoftheendothelialcelladhesionmoleculesthatmediateslymphocytebindingtotheendotheliumundershearconditions.Itisconstitutivelyexpressedmainlyinhighendothelialvenulesinperipherallymphnodes.TheexpressionofVAP-1isinducedbychronicinflammationinthevesselsofthetonsil,gut,skinandsynovium.VAP-1isalsopresentonsinusoidalandvascularendotheliumintheliverunderbothnormalandinflammatoryconditions,howeveritisnotfoundonanyofthebloodleukocytes.VAP-1expressionhasfurThermorebeenshowninhumancervixandvaginamucosa.Inpsoriasisandallergiclesions,VAP-1ismarkedlyupregulated.ThematureVAP-1moleculeisa170kDahomodimericglycoproteinthatconsistsoftwo90kDasubunitsheldtogetherbydisulfidebonds.VAP-1hasalargeextracellulardomain,asingle-passtransmembranedomain,andashortcytoplasmictail.Themoleculehasabundantsialicaciddecorationsthatareessentialtoitsadhesivefunction,becauseVAP-1isunabletomediatelymphocyteadhesiontodesialylatedvessels.TheleukocyteligandforVAP-1iscurrentlyunknown.InductionofVAP-1hasbeenshownatsitesofinflam-mation,suchasininflammatoryboweldiseasesandchronicdermatoses,whereexpressionofVAP-1isclearlyincreased.ItisconstitutivelyexpressedonhepaticendotheliumplayingacriticalroleinregulationofT-cellrecirculationtotheliver.StrongexpressionofVAP-1ontumorendotheliumdistinguisheshumanhepatocellularcarcinomasfromcolorectalhepaticmetastases.AcirculatingformofhumanVascularAdhesionProtein-1(sVAP-1)hasbeencharacterized.ThissVAP-1hasbeenshowntobeelevatedinseraofpatientswithcertainliverdiseasesandacorrelationwiththediagnosisofthepatientswasdemonstrated.
