Characteristicclinicalfeaturesare:

3.developmentoftransientheterophilandpersistentantibodyresponsesagainstEBV,
4.abnormalliverfunction.
4%ofinfectedyoungadultsshowanictericmanifestationand50%showasplenomegaly.InadditionEBVisimplicatedinBurkittlymphoma,thenasopharyngealcarcinomaandHodgkin´sdisease.
Aninfectiousmononucleosissimilarsyndromecanbecausedbycytomegalovirus,toxoplasmosisandotherviralinfections.Thereforedifferentialdiagnosisisofmajorimportance.SEROlogicaltestslikeEIAareveryusefulforthedetectionofanti-EBVIgGandIgMantibodiesespeciallywhenheterophilantibodiesareabsent.Relatedtofreshinfections,IgMantibodiesagainstVCAandEAaredeterminedbyimmunofluorescenceorELISA.LaterVCAIgGandthenEBNA-1IgGantibodiesappear.ThesimultaneousactivationofVCAIgMandEBNA-1IgGindicatesareactivationofalatentEBVinfection.
VCAIgGantibodiesoccurearlyindisease,reachthehighesttiterswithin2to4weeksandpersistformanyyearsprobablyforlife.VCAIgMantibodiesoccuratthebeginningofthediseasebutdecreaseorevendisappearwithinafewmonths.
