4%ofinfectedyoungadultsshowanictericmanifestationand50%havesplenomegaly.Inaddition,EBVisimplicatedinBurkittlymphoma,nasopharyngealcarcinomaandHodgkin´sdisease.
Aninfectiousmononucleosissimilarsyndromecanbecausedbycytomegalovirus,toxoplasmosisandotherviralinfections.ThereforeDifferentialdiagnosisisofmajorimportance.SEROlogicaltestslikeEIAareveryusefulforthedetectionofanti-EBVIgGandIgMantibodiesespeciallywhenheterophilantibodiesareabsent.Relatedtofreshinfections,IgMantibodiesagainstVCAandEAaredeterminedbyimmunofluorescenceorELISA.LaterVCAIgGandthenEBNA-1IgGantibodiesappear.ThesimultaneousactivationofVCAIgMandEBNA-1IgGindicatesareactivationofalatentEBVinfection.
