Reninisanenzyme(Mwof37kDa)thatbelongstotheasparticacidproteasefamily.ReninisamemberofRenin-Angiotensin-AldosteroneSystem(RAAS)thatcontrolsbloodpressure,renalbloodflux,glomerularfiltration,andsodium/potassiumhomeostasis.Reninisproducedconstitutivelyasprorenin,aninactiveprecursorwith386aminoacids,inthejuxtaglomerularcellsofthekidney.Inresponsetolowintra-renalbloodpressure,reducedsodiumreabsorption,hypokalemiaoractivityofthesympatheticnervoussystem,activerenincanbereleasedeitherfromadepotinthekidneyorgeneratedfromproreninbycleavageof46aminoacidsattheN-terminusofprorenin.Proreninsecretionintothebloodiscontinuous,incontrasttothetightlycontrolledreleaseofrenin,andbloodconcentrationofproreninisapprox.100-foldhigherthanactiverenin.

Afterreleaseandactivation,solublereninmediatescleavageofthea2-globulinangiotensinogenintotheprecursorpeptideangiotensinI,whichultimatelyisprocessedbyangiotensinconvertingenzyme(ACE)totheoctapeptideangiotensinII.AllactionsofangiotensinIIaremediatedbytheGprotein-coupledangiotensintype1(AT1)andangiotensintype2(AT2)receptors.DirectphysiologicaleffectsofAngiotensinIIincludevasoconstriction,increaseoftubularreabsorptionofsodiumandchloride,waterretention,andreleaseofthehormonesaldosteronefromadrenalcortex,antidiuretichormone(ADH,Vasopressin)fromposteriorpituitary,andadrenocorticotropichormone(ACTH,Corticotropin)fromanteriorpituitary.Releaseofthesehormonesfurthersupportssodiumretentionandsecretionofpotassium/H+inthekidney,andincreasesthirstsensationandthedesireforsaltthroughthesubfornicalorganofthebrain.Inanegativefeedbackloop,reninsecretionisreducedbyhighconcentrationofangiotensinII,andreleaseofaldosteroneisloweredbypotassiumdepletion.Besidetheactionofsolublerenin,bindingofreninandprorenintothemembrane-boundreninreceptorATP6AP2inbrain,heart,placenta,liver,kidneyandpancreasenhancesefficiencyofangiotensinogencleavageandinducesangiotensin-independentintracellulareffectsbyactivatingmitogenactivatedkinasesERK1andERK2.PlasmareninisagoodindexfortheactivityoftheRAAS.IncaseofdysfunctionoftheRAAS,theReninassaywillallowclinicalimplicationsfordiagnosis,treatment,andfollowup.
