ThemeasurementofProinsulininserumcanprovidevaluableinformationforthediagnosisofinsulinomas.Proinsulinlevelshavealsobeenshowntobeelevatedinnon-insulindependentdiabetics(NIDDM),newlydiagnosedinsulindependentdiabetics(IDDM)andotherclinicalsituations.Proinsulinisa9390MWpolypeptideof86aminoacids,thatissynthesizedintheßcellsofthepancreasandistheprecursormoleculeforinsulin(1,2,3).MostproinsulinisconvertedtoinsulinandC-Peptide,whicharesecretedinequimolaramountsintotheblood.About15%isnotconvertedandisreleasedasproinsulin.TheBIOLOGicalactivityofproinsulinisonlyabout10%ofInsulin,butthehalflifeofproinsulinisthreetimesaslongasinsulin.Thelevelofproinsulininserumcanbeareflectionofßcellstatus.BothIDDMandNIDDMarecharacterizedbydysfunctionofthepancreaticßcells.ElevatedproinsulinlevelshavebeennotedattheonsetofIDDMandinhealthysIBLingsofIDDMpatients.ProinsulinlevelsmayalsobeincreasedinpatientswithestablishedNIDDM.Increasedlevelsofcirculatingproinsulinarefoundinolderpatients,pregnantorobesediabetics,patientswithinsulinomas,functionalhypoglycemiaandhyperinsulinemia,araresyndrome.Becausethestructureofproinsulinissimilartoinsulin,proinsulinmaybedetectedasimmunoreactiveinsulinintheinsulinassay.ImmunoreactiveinsulinlevelsaregenerallydeterminedinconventionalRIA"s,whichoverestimatetheinsulinlevelbecausethemethodsuseantibodieswhichcrossreactwithproinsulin.Bycalculatingthemolarrationofproinsulintotrueinsulin(P/I),abetterassessmentofßcellfunctioncanbemade.
