TypeIDiabetesismainlycharacterisedbylimitedorfullymissingsecretionofthehormoneinsulin.Morphologicalstudiesdemonstratedadestructionofthebetacellsoftheso-calledLangerhanns"scheCells(IsletCells)inTypeIdiabetics.Numerousresearchersdescribedtheappearanceofantibodiesdirectedagainsttheisletcellsandinsulinasthecausalreasonfortheonsetofthedisease.Anti-Insulinantibodiesarefoundin37percentofpatientswithnewlydetectedTypeIDiabetes,in4percentoftheirrelativesofthefirstdegreeandinupto1,5percentofhealthycontrols.Apositivecorrelationbetweentheappearanceofanti-Insulinandanti-IsletCellantibodieshasbeenreported.Anti-Insulinautoantibodiesmaybedetectedseveralmonthsandinsomecasesyearsbeforetheonsetofthefullyclinicalmanifestationofthediseases.OccasionallyalsoautoantibodiestoPro-Insulinmayappear.These""true""anti-Insulinautoantibodiesdirectedagainstendogenousinsulinhavetobedis-tinguishedfromthoseautoantibodieswhicharedevelopedininsulindependentdiabeticsundergoingtherapywithinsulinpreparationsofanimalorigin.Infactthelatterhavetobereferredtosideeffects.Thesesideeffectsmayoccuraslocalreactionsoftheskinbydevelopmentofinsulin-specificautoantibodies.Theseautoantibodiesarecausingtheformationofaninsulindepotandtheymaysimulatearesistanceagainstthehormonaltreatmentwithanimalinsulin.AdditionallyotherimmunologicalphenomenonhavebeenreportedforTypeIdiabetics.Alotofotherautoantibodyspecificitieshavebeendetectedinthosepatientstoo,buttheseantibodiesmustnotcauseadditionalautoimmunephenomenon.
