ProgesteroneisaC-21steroidhormone(molecularweight:314.5)whichissynthesizedfromcholesterolviapregnenoloneinthegranulosaandthecacellsofthecorpusluteumundertheinfluenceofLH.Themajorproductionsitesareovaryandplacentaandsomewhattheadrenalcortexinbothmenandwomen.Progesteroneisrapidlymetabolizedintheliver.Bloodlevelsareverylowduringthefollicularphasewhereasonedoesobserveasharplyincreaseduringthelutealphaseofmenstrualcyclesreachingamaximumsome5to10daysafterthemidcycleLHpeak.
Clinicalapplications
Serumprogesteronelevels,whicharelowduringthefollicularphase,increaseduringthelutealphaseofmenstrualcycle.Unlesspregnancyoccurs,theprogesteroneleveldeclines4daysbeforethenextmenstrualperiod.Thus,themeasurementofprogesteronelevelsconstitutesawell-establishedmethodfordetectionofovulation.Buttherearemanycaseswheretheprogesteronemeasurementsarealsoofinterest:
- Tochecktheeffectivenessofovulationinduction;
- Tomonitortheembryotransferandprogesteronereplacementtherapy;
