DHEA(Dehydroepiandrosterone)anditssulphateesterDHEA-S,asC-19steroidhormones,belongtothegroupofandrogens.Astheyarealmostexclusively(96%)producedintheadrenalcortex,theyarethemostimportantadrenalandrogens.Additionally,theyarealsoproducedinthegonadsandinthebrain.
Duetoquantityproduced,theycanbeconsideredasthemainproductsofhumansteroidbiosynthesis.
DHEA-Scirculatesinbloodin20-foldhigherconcentrationsthananyotherhormone.TheconcentrationofDHEA-SexceedstheDHEAlevelsbyapproximately300to500times.ItservesasakindofrepositoryformofDHEA.TheconversionofDHEA-SintoDHEAtakesplaceveryquicklyandinalmostallbodytissuesasthecorrespondingenzymesareavailableubiquitously.

ThenormalDHEAconcentrationchangessignificantlywithage:AtbirththeDHEAvaluesareveryhigh,droppingsharplytobarelymeasurablevalues,andremainatlowlevelsuntilthe7thto10thyearoflife,whentheadrenalcortexmaturesandDHEAvaluesbegintoriseagain.Inadults,peakconcentrationsofDHEAwithinanindividualareexpectedtobeachievedbetweenthe25thandthe35thyear,followedbyaslowbutcontinuousdeclineuntilalowvaluearoundthe50thyearandfinallyreaching,atadvancedage,when10-20%ofthepeakvaluesareobserved.Thisage-relateddeclineofDHEAsecretionisknownas“Adrenopause”andistheresultofareductionoftheproductionofDHEAintheadrenalcortex.
InserumandinsalivaDHEAvaluesareobservedtobepulsatileaswithothersteroidhormonewithclearoscillationrangearoundthemiddlevalue.Duetothepulsatiledynamicsofsteroidsecretionrepeatedsalivasamplingisrecommended.
