Theplasmaconcentrationoftranscortinshowslittleornodiurnalvariationandnomarkeddifferencesareobservedinadultsubjectsaccordingtoage,sexormenstrualcycle.Inumbilicalcordblood,however,transcortinispresentathalfofthenormaladultlevelandprepubertalchildrenhavesomewhathigherlevelsthanadults.Estrogentherapyorestrogenimpregnationduringpregnancycausesaverymarkedincreaseofthetranscortinconcentration.Decreasedlevelsoftranscortinareobservedinseveralconditions:
hypoproteinemia,Cushing"ssyndromeorcorticoïdtreatmentandinsomecasesofvitaminB12deficiency.
Extremelylowlevelsoftranscortinhavebeenreportedinafewpatientswithsepticshock.FurThermore,arareinheritedformofhypotranscortinemiahasbeendescribed.
Themostimportantclinicalapplicationoftranscortinmeasurementsconsistsoftheinterpretationofcortisollevels,sinceitallowstoassesstheunboundcortisolconcentration,whichisBIOLOGicallyactive.Indeed,theconcentrationofunboundcortisolcanbecalculatedfromtheconcentrationoftotalcortisolandthatoftranscortinonthebasisofmassaction.Theresultsofthismethodcorrelatewellwiththoseobtainedbycentrifugedultrafiltrations.
