LeptospirosisisanacutetochroniczoonoticdiseasecausedbypathogenicspeciesofLeptospira.Thesourcesofinfectionarerodents,mainlyratsandmice.Theanimalsexcretethepathogensintheurine.Theycanenterthehumanbodythroughsmallcutsintheskin.Thenumberofnewcasesworldwideisestimatedat300,000to500,000cases.Majoroutbreakshavebeenobservedinparticularinthesettingofflooding.Intheindustrializednations,localoutbreaksoccurprimarilyinthecontextofrecreationalactivitiessuchaswatersportsorboattrips.Approximately90%ofpatientshavemildflu-likesymptoms.Theseverecourseofdisease,alsoknownasWeil"sdisease,ischaracterizedbybleeding,renalfailure,jaundiceandfatalcomplications.
Pathogenscanberecoveredfrombloodculturesonlyinthefirstphaseofthedisease.Theculturetakesquitealongtimeaswell,meaningthatasuccessfulantibiotictreatmentcannotbeinstitutedinatimelymanner.Forthisreason,serologicalmethodsareincreasinglyused.Theserologicalreferencemethodisthemicroagglutinationtest,which,however,istime-consumingandcarriesariskofinfectionbecauseitemployslivecultures.ThisaccountsforthespecialsignificanceoftheELISAmethodforthedeterminationofspecificantibodies,whichcanbedetectedwithin5-7daysaftertheonsetofclinicalsymptoms.ThepresenceofspecificIgMantibodiesisconclusiveforacuteillness.
