Insulin-likegrowthfactorbindingprotein-1(IGFBP-1)isoneofsixproteinsthatspecificallybindinsulin-likegrowthfactorsIandII(IGF-IandIGF-II)inbodyfluidsandtissues.IFGBP-1contains234aminoacids,withapredictedmolecularmassof25kDa.ThemajorsitesofIGFBP-1synthesisarethefetal/adultliveranddecidualizedendometrium.SerumlevelsofIGFBP-1,whichreflectitssynthesisbytheliver,exhibitconsiderablediurnalvariation.CirculatingIGFBP-1levelsarehighestearlyinthemorningandlowestintheevening.Thelevelsarehighinthefetusandnewborn,butdeclinesteADIlyuntilpuberty.ThemeanlevelofIGFBP-1inhealthyadultsis4.4µg/L(range0.6-14.4µg/L).Afterabout65yearsofage,serumIGFBP-1levelsbegintoincrease.Thereisalsoaninversecorrelationbetweenbodymassindex(BMI)andfastingserumIGFBP-1concentrations.ThemostimportantregulatorofcirculatingIGFBP-1isinsulin.FastinginsulinandIGFBP-1concentrationsareinverselycorrelated.Duringa3-hglucosetolerancetest,thereisadecreaseofabout50%inserumIFGBP-1levels.Eatingamealalsohasadecreasingeffect.Ininsulin-dependentdiabetes(IDDM),serumIGFBP-1levelsareelevated.Innon-insulindependentdiabetes,inwhichinsulinlevelsarehigh,serumIGFBP-1isdecreased.LowlevelsofIGFBP-1havealsobeenobservedinthefollowingcases:acromegaly,Cushing’ssyndromeandpolysysticovariansyndrome(PCO).
