Catecholaminesandtheirmetabolites,metanephrineandnormetanephrine,aresecretedinexcessiveamountbyvarioustumors,includingparaganglioma,neuroblastoma,andinparticularpheochromocytoma.TheclinicalmanifestationofthesetumorsishighlyvariableandoftenresemblessymptomsassociatedwithcommonmalADIes,predominantlyhypertension,headacheandsweating.
Biochemicaltestingforpheochromocytomaisindicatednotonlyinsymptomaticpatients,butalsoinpatientswithadrenalincidentalomasoridentifiedgeneticpredispositions.Itinvolvesmeasurementsofcatecholamines(adrenaline,noradrenalineanddopamine)andtheirmetabolites(metanephrine,normetanephrineandvanillylmandelicacid)inplasmaorurine.
NumerousstudieshaveshownthatfreemetanephrinesinplasmaarethebestMarkersforthediagnosisofpheochromocytomes(Grossmannetal,2006;Lendersetal.,2005;Eisenhoferetal.,2000)

Enzymeimmunoassayforthequantitativedeterminationoffreemetanephrineandnormetanephrineinhumanplasma.Thecatecholaminesadrenalin,noradrenalinanddopaminearesynthesizedintheadrenalmedulla,thesympatheticnervoussystemandinthebrain.Theyinfluencevirtuallyalltissuesandareinvolvedtogetherwithotherhormonalandneuronalsystemsintheregulationofawidevarietyofphysiologicalprocesses.
Ascatecholaminesandtheirmetabolitesmetanephrineandnormetanephrinearesecretedinincreasingamountsinanumberofdiseases,theymaybeusedfordiagnosticpurposes.
Inthiscontext,diagnosisaswellasthefollow-upoftumordiseasesofthenervoussystemareofspecialimportance.Thisappliesprimarilytothepheochromocytoma,butalsotheneuroblastomaandtheganglioneuroma.
Malignantgrowthisdescribedin10%ofpheochromocytomas.FurThermore,anincreaseofcatecholaminesandtheirmetabolitesmetanephrineandnormetanephrinecanbeobservedinthecarcinoid.
Dataofthelastyearshaveshownthatthemetanephrinesinplasmaarethebestmarkerfordiagnosisandfollow-upofpheochromocytoma.
