Calcitoninisa32-amino-acidpolypeptidethatissecretedprimarilybytheparafollicularC-cells.ItsmainBIOLOGicaleffectistoinhibitosteoclasticboneresorption.ThispropertyhasledtoCalcitonin’susefordisorderscharacterizedbyincreasedresorptionsuchasPaget’sdiseaseandforsomepatientswithosteoporosis.Theaminoacidsequencesofporcine,bovine,salmon,andhumanCalcitoninareknown,andbiologicallyactivemoleculeshavebeensynthesized.Thestructureofeachissimilar,whereastheimmunologicactivityofCalcitoninfromvariousspeciesdiffersprofoundly.ThemostprominentclinicalsyndromeassociatedwithadisorderedhypersecretionofCalcitoninismedullarycarcinomaofthethyroid(MTC).MTCisatumoroftheCalcitoninproducingC-cellsofthethyroidgland.MTCcomprises5-10%ofallthyroidcancer.ItmayoccursporADIcallyorinafamilialformthatistransmittedasanautosomaldominanttrait.Despiteitsbeingarelativelyuncommontumor,MTChasgreatclinicalimportancebecauseofitsfamilialdistribution.Further,itlentitselftobediagnosedearlybyserumCalcitonin.Thisisfrequentlyassociatedwithotherclinicalfeaturesandithasgoodpotentialforcurewithsurgery.Althoughararetumor,itcanoccurinafamilialpatternasaTypeIImultipleendocrineneoplasia.ThesetumorsusuallyproducediagnosticallyelevatedserumconcentrationsofCalcitonin
