Insulin-likegrowthfactorI(IGF-I)isapolypeptideof70aminoacids(7650daltons),andisoneofanumberofrelatedinsulin-likegrowthfactorspresentinthecirculation.Themoleculeshowsapproximately50%sequencehomologywithproinsulinandhasanumberofbiologicalactivitiessimilartoinsulin.Thepeptideisgrowthhormone(GH)dependenttoahighdegree,butthereisgrowingevidenceofGH-independentsecretion.IGF-Ihasnumerousgrowth-promotingeffects,includingmitogeniceffectsandthepromotionofcartilagesulphation.Italsomediatesgrowthpromotingactionsofgrowthhormoneonskeletalandotherbodyfluids.Almostall(>95%)ofserumIGF-IcirculatesboundtospecificIGFbindingproteins,ofwhichsixclasses(IGF-BPs1-6)arenowrecognised.BP3isthoughttobethemajorbindingproteinofIGF-I,formingaternarycomplexof140000daltonswithIGF-Iandanacid-lABIlesub-unit.ThemeasurementofserumIGF-Iisofrecognisedvalueinchildrenwithgrowthdisordersandinthediagnosisandmonitoringofacromegaly.IGF-Iconcentrationschangewithage,nutritionalstatus,bodycompositionandgrowthhormonesecretion.AsinglebasalIGF-Ideterminationisusefulintheassessmentofshortstatureinchildrenandinnutritionalsupportstudiesofacutelyillpatients.Forthediagnosisofacromegaly,asingleIGF-IdeterminationisconsideredmorereliablethanarandomGHdetermination.
