BileacidsareC24steroidsderivedfromcholesterol.Cholicandcheno-deoxycholicacidsarethetwoprimarybileacidsinman.Inbile,theyareconjugatedwiththeaminoacids,glycineandtaurine,formingthefourconjugatedbileacidswhicharemeasuredbythisassay:glycocholicacid,taurocholicacid,glycochenode-oxycholicacidandtauRochenodeoxy-cholicacid.Theprimarybileacidsundergobacterialdegradationintheintestinetoformthesecondarybileacids,deoxycholic,lithocholicacidsandoxidationproducts.Thesaltsoftheconjugatedbileacidsarepowerfuldetergentswhichaidinintestinalabsorptionoflipids.Theyarealmostentirelyabsorbedbytheintestineandreturnedtotheliverandgallbladderviatheenterohepaticcirculation.Normally,lessthan1%ofthebileacidpoolisfoundinsystemicblood.Ithasbeenknownformanyyearsthattheconcentrationofbileacidsinserumisincreasedinpatientswithliverdiseaseasaresultofthefailureofthediseasedlivertoextractbileacidsefficientlyfromportalblood1.Serumconjugatedbileacidconcentrationshavebeenreportedtoberaisedinmanyformsofstructuralliverdisease.Metabolichepaticdiseasesinvolvingorganicanionsdonotseemtocauseabnormalbileacidconcentrations2.Noincreaseinserumconjugatedbileacids,eveninthepresenceofliverdisease,willbenotedinpatientswithintestinalmalabsorptionsuchasisfoundfollowingilealresectionorby-pass2,3.Ithasbeensuggestedthatbileacidlevelsmaybethesinglemostsensitiveandspecificindicatorofliverdiseasewhereotherchemicalandenzymetestsareoftennormal2,4-9,suggestingapplicationforfollowingrecoveryorpredictingrelapseindiseasessuchashepatitisandformonitoringdrugtoxicity.
