TheIBLIA2-GAD65ScreenELISAisasolidphaseenzymeimmunoassayemployingafusionproteinofrecombinanthumantyrosinephosphatase-relatedproteinisletantigen2(IA2)andrecombinanthumanglutamatedecarboxylase65kDaisoform(GAD65)forthequantitativeandqualitativedetectionofantibodiesagainstisletcellsinhumanserum.Theassayisatoolinthediagnosisoftype1diabetesmellitus.Theinsulindependentdiabetesmellitus,alsocalledtypeIdiabetes(T1DM),isachronicautoimmunediseaseresultingfromtheautoimmunedestructionofinsulinproducingß-cellsofLangerhans`islets.Itischaracterizedbylargelyorcompletelackofinsulinproductionandthepresenceofß-cellautoantibodies.Theantigensrecognizedbytheseantibodiesincludeinsulin,glutamatedecarboxylase(GAD65kDaisoform)andtyrosinephosphatase-relatedproteinisletantigen2(IA2).AutoantibodiesagainstisletcellantigensareimportantpreclinicalMarkersastheymaybepresentforyearsbeforediagnosisofdiabetesandatatimewhereothermetabolicassaysshownormaldiagnosticfindings.RecentstudiesshowthatacombinedtestingforantibodiesagainstGAD65andIA2detectsmorethan90%ofnewonsettype1diabetespatients.TheIBLELISAIA2-GAD65ScreenallowsthesimultaneousdetectionofautoantibodiesagainstGAD65andIA-2.It’sanidealscreeningassaytoidentifyindividualsofhighriskfortype1diabetesmellitus.
